Thomas Smith

Thomas Smith is a Board-Certified Psychiatric Mental Health Nurse Practitioner with clinical experience in behavioral health spanning inpatient, residential, and community-based settings. He has worked in inpatient and residential programs at McLean Hospital, where he gained experience treating acute psychiatric conditions, complex mood disorders, and severe mental illness.

Thomas has also worked extensively in community mental health, providing care to individuals with chronic and persistent mental illness. In addition to direct patient care, he has experience developing, launching, and managing a stand-alone community psychiatric unit, with responsibilities that included clinical oversight, program development, and operational management.

He earned a Bachelor's degree from Boston College, a Master's degree in Healthcare Administration from Framingham State University, and a Master's degree in Nursing from Regis College. Thomas specializes in the assessment and treatment of ADHD, mood disorders, substance use disorders, and men's mental health concerns. His clinical approach integrates evidence-based pharmacologic treatment with behavioral interventions, emphasizing functional improvement, accountability, and long-term stability.

RMV Evaluations with Thomas Smith

Thomas completes RMV-required evaluations for adults by secure video or in person at our Medway office. That includes the RMV Substance Abuse Evaluation with Risk of Recidivism required after an OUI, DUI, Immediate Threat or ignition interlock case, and the RMV Psychiatric Evaluation (Form MAB118) requested by RMV Medical Affairs.

Substance use is one of Thomas's clinical specialties, and his experience in residential, inpatient and community settings informs the substance use history, treatment review and clinical opinion at the center of every substance abuse evaluation and risk of recidivism assessment. Massachusetts requires Form MAB118 to be completed by a licensed physician or a psychiatric nurse practitioner, and Thomas is a board-certified psychiatric mental health nurse practitioner (PMHNP-BC).

Our RMV evaluations are approved by the Massachusetts RMV and can be completed in as little as 48 hours when a hearing or deadline is close. Results are reported directly to the RMV, and we coordinate dates and documents with your attorney. RMV evaluations are self-pay.
