RMV Evaluations in South Shore

If your license was suspended or revoked after an OUI or DUI, an Immediate Threat determination or an ignition interlock violation, the RMV will usually require a clinical evaluation before a hearing officer will consider reinstatement. Greater Boston Psychiatric Services completes these evaluations for drivers throughout the South Shore.

Most clients complete their evaluation by secure video from home. If you'd rather meet face to face, our office is at 165 Main Street, Suite 111A, Medway, MA 02053.

That includes drivers in Brockton and Quincy.

We also serve Braintree, Weymouth, Randolph, Milton, Stoughton, Bridgewater, Hingham, Hanover, Marshfield, Scituate, Duxbury, Plymouth, Abington, Rockland and Whitman, and every other community in the region.

Courts and RMV Offices in South Shore

OUI cases in the South Shore are heard at the district court for the town where the stop happened, including:

  • Quincy District Court

  • Brockton District Court

RMV Service Centers in the South Shore include:

  • Braintree RMV Service Center

  • Brockton RMV Service Center

  • Plymouth RMV Service Center

RMV suspension hearings are currently held by phone, so you don't need to travel for your hearing. The RMV is separate from the courts and may have its own requirements, including this evaluation, even after your court case is resolved.

Which RMV Evaluation Do You Need?

Your RMV letter or hearing notice lists your requirements. We offer both evaluations the RMV typically asks for, separately or together:

  • Substance Abuse Evaluation with Risk of Recidivism: the most common requirement after an OUI, DUI or Immediate Threat suspension.

  • RMV Psychiatric Evaluation (Form MAB118): required when the RMV has concerns about mental health or medications.

  • Both, combined: one appointment at the combined rate.

Not sure what your notice is asking for? Include it with your intake form and we'll confirm before you book.

What Is a Risk of Recidivism Assessment?

A risk of recidivism assessment is the clinician's written opinion on how likely you are to have another alcohol- or drug-related driving offense. It is attached to your substance abuse evaluation, which is why many people call it a "letter of recidivism." It draws on your substance use history, any prior offenses, the treatment and recovery steps you've taken, and your mental health history.

The RMV does not waive this requirement. Until it is completed, your suspension can remain in place indefinitely, so it's worth getting it done early.

Who We Evaluate

  • First OUI or DUI, as part of reinstatement

  • Second or subsequent OUI, including cases involving ignition interlock

  • Immediate Threat suspensions involving alcohol, illegal drugs or prescription medication

  • Ignition interlock violations

  • Out-of-state offenses that affect your Massachusetts license

How the Process Works

  1. Complete the quick form. Include your hearing date and your attorney's name if you have one.

  2. Meet with your clinician. A one-hour interview by secure video, or in person at our Medway office.

  3. We complete your report. Your substance abuse evaluation and risk of recidivism assessment.

  4. You receive your written report, which you can provide to the RMV so you can move forward with your hearing.

Turnaround: in as little as 48 hours when your hearing is close. Tell us your date and we'll prioritize accordingly.

What the Evaluation Assesses

  • Alcohol and drug use history, including current use or abstinence

  • Prior offenses and driving history

  • Past and current treatment, support groups and recovery steps

  • Mental health and medical history relevant to driving safety

  • Standardized screening tools

  • A risk of recidivism opinion, with recommendations when clinically appropriate

How to Prepare

  • Have your RMV letter or hearing notice on hand.

  • Gather any court paperwork, treatment records or program completion letters.

  • For a video appointment, find a private space with a stable internet connection and a device with a camera.

  • Answer openly. The evaluation looks at the whole picture, and it is only as useful as the information it is based on.

Because our clinical team includes multiple board-certified psychiatric nurse practitioners and clinical nurse specialists, we can often offer earlier appointments than a practice that depends on a single provider's schedule.

Other RMV Requirements You May Have

Immediate Threat cases often also require a medical evaluation or a loss of consciousness form completed by your primary care provider. RMV suspension hearings are currently held by phone and booked through the RMV Online Service Center. Chemical test refusal hearings are the exception: they are held in person at the Boston (Haymarket) RMV Service Center. Official forms are available on the Mass.gov RMV forms page.

Pricing

  • Substance Abuse Evaluation with Risk of Recidivism: $450

  • Substance Abuse Evaluation + RMV Psychiatric Evaluation: $525

This evaluation is self-pay only. RMV-required evaluations are a legal requirement rather than medical treatment, so we are not able to bill insurance, Medicare or Medicaid. Payment is collected at the time of your appointment. See our Good Faith Estimate.

Frequently Asked Questions

Can I complete the evaluation by video?

Yes, you can complete the evaluation by video. The clinical content and reporting are the same whether you attend by video or in person; confirm any acceptance requirements with the RMV, the court or your attorney before you schedule.

In as little as 48 hours when your hearing is close. Expedited scheduling is available for an additional $225. Let us know your hearing date when you contact us.

Do I have to travel to be evaluated?

No. Adults anywhere in the South Shore can complete the entire evaluation by secure video. In-person appointments are available at our Medway office.

Do I need both a substance abuse evaluation and a psychiatric evaluation?

Some RMV cases require only one; others require both. Check your RMV notice, or send it to us and we'll confirm before you schedule. A combined rate applies if you need both.

Can you evaluate ignition interlock violations?

Yes. We complete RMV evaluations for interlock violations as well as OUI, DUI and Immediate Threat cases.

What happens to the results?

Following your evaluation you receive a written report to provide to the RMV, a court or your attorney, as required.

Does completing this evaluation guarantee my license will be reinstated?

What if I've moved out of Massachusetts since my case began?

As long as you're physically located in Massachusetts at the time of your appointment, we can complete the evaluation. If you've relocated permanently, contact us to confirm eligibility.

Is what I share confidential?

Yes. Your evaluation is handled under the same clinical confidentiality standards as any other appointment, with the exception of a written report you may need to provide to the RMV.

Crash and Impaired Driving Statistics in South Shore

The five-year MassDOT crash count for South Shore (2021 to 2025) is 73,174. That includes 2,118 crashes with suspected alcohol, 511 with suspected drug use and 207 fatal crashes. These figures add up MassDOT's records for 27 towns in the region.

Data come from the MassDOT IMPACT Crash Data Portal (accessed October 2026). Suspected alcohol and drug flags depend on what was recorded at the scene, so they probably understate impaired driving, and crashes are not the same as OUI charges.
