RMV Psychiatric Evaluations for Buzzards Bay Drivers

If RMV Medical Affairs has asked you to complete a Psychiatric Evaluation (Form MAB118), Greater Boston Psychiatric Services can complete it for adults in Buzzards Bay. Buzzards Bay drivers include residents of Buzzards Bay village, Monument Beach, Pocasset, Gray Gables, Cataumet and Sagamore.

Buzzards Bay clients can meet with us by secure video from home, which means no drive to Medway. If you would rather meet face to face, our office is at 165 Main Street, Suite 111A, Medway, MA 02053.

Buzzards Bay Village, Bourne and the Canal

Buzzards Bay sits at the head of the bay of the same name, at the Cape Cod Canal's western end. The Bourne Bridge and the Sagamore Bridge carry traffic over the canal, and those crossings are what make a trip to Medway a real commitment for Upper Cape residents. That is why we built our RMV evaluations around secure video: you can complete the evaluation from home in Buzzards Bay, Pocasset or Cataumet without planning around bridge traffic.

If your RMV notice or a letter from your attorney lists a Bourne address, the Town of Bourne and Buzzards Bay are the same jurisdiction for court purposes. Use whichever name appears on your paperwork when you complete our intake form.

Local Courts and RMV Offices for Buzzards Bay Drivers

Form MAB118 is handled by RMV Medical Affairs, which reviews completed forms centrally rather than through a local office. Buzzards Bay is a village of the Town of Bourne, so its drivers are under Bourne's court and RMV jurisdiction. There is an RMV Service Center in South Yarmouth on the Cape, and another in Plymouth across the canal. Hearings for suspensions are held by phone, so you do not need to travel to an RMV office for them.

A Medical Affairs request is separate from any court case. You can be asked for this evaluation even if you have never been charged with anything.

What Is the RMV Psychiatric Evaluation (Form MAB118)?

RMV Medical Affairs requests a Psychiatric Evaluation (Form MAB118) when it has received a report, from a health care provider or law enforcement official, suggesting that a driver may have a psychiatric or medical condition affecting their ability to drive safely. It is a different requirement from the substance abuse evaluation with risk of recidivism: it is not tied to an OUI or DUI case.

Massachusetts requires the form to be completed by a physician licensed in the state or a psychiatric nurse practitioner, which are the credential types on our clinical team.

How the Process Works

  1. Check your notice. Read the request you received from RMV Medical Affairs and note any deadline it gives.

  2. Complete the quick form. Include any deadline on your notice and your attorney's name if you have one.

  3. Meet with your clinician. By secure video, or in person at our Medway office.

  4. We submit the completed form to RMV Medical Affairs.

Turnaround: in as little as 48 hours when your deadline is close. Tell us your date and we'll prioritize accordingly.

What the Evaluation Assesses

  • The specific condition or concern named in the RMV's request

  • Current symptoms and how they may affect safe driving

  • Relevant treatment history and current treatment status

  • Current medications and how they affect alertness and judgment

  • A clinical judgment on fitness to operate a motor vehicle, per the form's requirements

Frequently Asked Questions

How is this different from the RMV substance abuse evaluation?

The substance abuse evaluation with risk of recidivism relates to OUI and DUI cases. Form MAB118 is requested by RMV Medical Affairs whenever it receives a report suggesting a psychiatric or medical condition may affect your ability to drive safely.

Who is allowed to complete Form MAB118?

Massachusetts requires the form to be completed by a physician licensed in Massachusetts or a psychiatric nurse practitioner, both credential types on our clinical team.

Do I have to travel from Buzzards Bay for this evaluation?

No. Buzzards Bay residents can complete the evaluation by secure video, or in person at our Medway office if you prefer.

Is a video evaluation accepted?

Yes. The clinical content and the completed form are the same whether you attend by video or in person at our Medway office.

How soon can I get an appointment?

In as little as 48 hours when your deadline is close. Expedited scheduling is available for an additional $225.

What if RMV Medical Affairs has follow-up questions?

Medical Affairs may follow up with you directly or request more information. We're glad to complete a follow-up evaluation if one is requested.

Is this evaluation confidential?

Your evaluation follows standard clinical confidentiality practices, with the exception of the completed form we are required to submit to RMV Medical Affairs.

Impaired Driving Data for Buzzards Bay

From 2021 to 2025, Bourne had 3,311 reported crashes. 117 were flagged for suspected alcohol, 32 for suspected drug use, and 7 were fatal. Buzzards Bay is a village within Bourne, and MassDOT reports crashes by town, so these are the Bourne figures.

Data come from the MassDOT IMPACT Crash Data Portal (accessed October 2026). Suspected alcohol and drug flags depend on what was recorded at the scene, so they probably understate impaired driving, and crashes are not the same as OUI charges.

A Form MAB118 evaluation is usually prompted by a report that a driver may have a condition affecting safe driving. Our article, Recidivism, Mental Health and RMV Evaluations in Massachusetts, explains the RMV process and what the research says about OCD. See also the conditions we treat.
