Courts and RMV Offices in The Berkshires

RMV Service Centers in the Berkshires include:

  • Pittsfield RMV Service Center

  • North Adams RMV Service Center

Form MAB118 is reviewed centrally by RMV Medical Affairs, so your local service center is mainly relevant for license transactions after a decision is made.

What Is the RMV Psychiatric Evaluation (Form MAB118)?

RMV Medical Affairs requests a Psychiatric Evaluation (Form MAB118) when it has received a report, from a health care provider or law enforcement official, suggesting that a driver may have a psychiatric or medical condition affecting their ability to drive safely. It is a different requirement from the substance abuse evaluation with risk of recidivism: it is not tied to an OUI or DUI case, and it can be requested whenever Medical Affairs has a safety concern about a driver's mental health.

Massachusetts requires the form to be completed by a physician licensed in the state or a psychiatric nurse practitioner, which are the credential types on our clinical team.

How the Process Works

  1. Check your notice. Read the request you received from RMV Medical Affairs and note any deadline it gives.

  2. Complete the quick form. Include any deadline on your notice and your attorney's name if you have one.

  3. Meet with your clinician. By secure video, or in person at our Medway office.

  4. We submit the completed form to RMV Medical Affairs, consistent with the RMV's own process.

Turnaround: in as little as 48 hours when your deadline is close. Tell us your date and we'll prioritize accordingly.

What the Evaluation Assesses

  • The specific condition or concern named in the RMV's request

  • Current symptoms and how they may affect safe driving

  • Relevant treatment history and current treatment status

  • Current medications and how they affect alertness and judgment

  • A clinical judgment on fitness to operate a motor vehicle, per the form's requirements

How to Prepare

  • Have your RMV Medical Affairs letter and the MAB118 form on hand.

  • Gather any recent treatment records or a list of your current medications.

  • For a video appointment, find a private space with a stable internet connection and a device with a camera.

  • Answer openly. The evaluation is only as useful as the information it is based on.

Because our clinical team includes multiple board-certified psychiatric nurse practitioners and clinical nurse specialists, we can often offer earlier appointments than a practice that depends on a single provider's schedule.

After Your Evaluation

We submit the completed form to RMV Medical Affairs, so you won't need to forward anything yourself. Medical Affairs then reviews it as part of its own determination about your driving privileges. Processing time on the RMV's end is outside our control, so schedule with enough runway before any deadline.

Pricing

  • RMV Psychiatric Evaluation (Form MAB118): $450

  • Psychiatric Evaluation + Substance Abuse Evaluation: $525

Payment

This evaluation is self-pay only. RMV-required evaluations are a legal requirement rather than medical treatment, so we are not able to bill insurance, Medicare or Medicaid. Payment is collected at the time of your appointment. See our Good Faith Estimate.

Frequently Asked Questions

How is this different from the RMV substance abuse evaluation?

The substance abuse evaluation with risk of recidivism relates to OUI and DUI cases. Form MAB118 is requested by RMV Medical Affairs whenever it receives a report, not necessarily related to alcohol or drugs, suggesting a psychiatric or medical condition may affect your ability to drive safely.

Who is allowed to complete Form MAB118?

Massachusetts requires the form to be completed by a physician licensed in Massachusetts or a psychiatric nurse practitioner, both credential types on our clinical team.

Do I have to travel for this evaluation?

No. Adults anywhere in the Berkshires can complete the evaluation by secure video, or in person at our Medway office.

Is a video evaluation accepted?

Yes. The clinical content and the completed form are the same whether you attend by video or in person at our Medway office.

How soon can I get an appointment?

In as little as 48 hours when your deadline is close. Expedited scheduling is available for an additional $225.

Can I request a copy of the completed form?

Yes. You can request a copy for your records; the original is still submitted to RMV Medical Affairs as required.

What if RMV Medical Affairs has follow-up questions?

Medical Affairs may follow up with you directly or request more information. We're glad to complete a follow-up evaluation if one is requested.

Will you work with my attorney?

Yes. List your attorney on the intake form and we'll coordinate dates and documents.

Is this evaluation confidential?

Your evaluation follows standard clinical confidentiality practices, with the exception of the completed form we are required to submit to RMV Medical Affairs.

Impaired Driving Data for the Berkshires

From 2021 to 2025, the Berkshires had 11,057 reported crashes. 366 were flagged for suspected alcohol, 88 for suspected drug use, and 36 were fatal. These figures add up MassDOT's records for 9 towns in the region.

Data come from the MassDOT IMPACT Crash Data Portal (accessed October 2026). Suspected alcohol and drug flags depend on what was recorded at the scene, so they probably understate impaired driving, and crashes are not the same as OUI charges.

A Form MAB118 evaluation is usually prompted by a report that a driver may have a condition affecting safe driving.
