What This Evaluation Covers
The Massachusetts RMV requires a Psychiatric Evaluation (Form MAB118) when its Medical Affairs unit has received a report — from a physician, family member, or law enforcement official — suggesting that a driver may have a psychiatric or medical condition affecting their ability to operate a vehicle safely. This is a different requirement from the RMV's substance abuse evaluation with risk of recidivism assessment: it isn't tied to an OUI/DUI case, and it can be requested at any point Medical Affairs has a safety concern about a driver's mental health.

Schedule your appointment. Complete our registration form online; appointments are available by telehealth to any adult located in Massachusetts.
Complete the evaluation by video. Massachusetts requires this form to be completed by a physician licensed in the state or a psychiatric nurse practitioner — the credential types on our clinical team.
We submit the completed form to RMV Medical Affairs. The completed form is mailed or faxed directly to the RMV's Driver Control Unit / Medical Affairs office, consistent with the RMV's own process.
