It is a different requirement from the substance abuse evaluation with risk of recidivism: it is not tied to an OUI or DUI case, and it can be requested whenever Medical Affairs has a safety concern about a driver's mental health.

 

Massachusetts requires the form to be completed by a physician licensed in the state or a psychiatric nurse practitioner, which are the credential types on our clinical team.

 

How the Process Works

  1. Check your notice. RMV Medical Affairs sends Form MAB118 directly and states that a completed evaluation is required.

  2. Complete the quick form. Include any deadline on your notice and your attorney's name if you have one.

  3. Meet with your clinician. By secure video, or in person at our Medway office.

  4. We submit the completed form directly to RMV Medical Affairs, consistent with the RMV's own process.

We submit the completed form directly to RMV Medical Affairs, so you won't need to forward anything yourself.
