RMV Evaluations for Warren Drivers
If your license was suspended or revoked after an OUI or DUI, an Immediate Threat determination or an ignition interlock violation, the RMV will usually require a clinical evaluation before a hearing officer will consider reinstatement. Greater Boston Psychiatric Services completes these evaluations for drivers throughout Warren, including Warren center, West Warren and neighboring Brimfield, Palmer and West Brookfield.
Warren clients typically complete their evaluation by secure video, which avoids a long drive. If you'd rather meet face to face, our office is at 165 Main Street, Suite 111A, Medway, MA 02053.
Local Courts and RMV Offices for Warren Drivers
OUI cases arising in Warren are heard at East Brookfield District Court.
The closest RMV Service Center for Warren drivers is in Southbridge.
The RMV is separate from the courts. Even after your Warren court case is resolved, the RMV may have its own requirements, including this evaluation, before your license is reinstated.
Which RMV Evaluation Do You Need?
Your RMV letter or hearing notice lists your requirements. We offer both evaluations the RMV typically asks for, separately or together:
Substance Abuse Evaluation with Risk of Recidivism: the most common requirement after an OUI, DUI or Immediate Threat suspension.
RMV Psychiatric Evaluation (Form MAB118): required when the RMV has concerns about mental health or medications.
Both, combined: one appointment at the combined rate.
Not sure what your notice is asking for? Include it with your intake form and we'll confirm before you book.
What Is a Risk of Recidivism Assessment?
A risk of recidivism assessment is the clinician's written opinion on how likely you are to have another alcohol- or drug-related driving offense. It is attached to your substance abuse evaluation, which is why many people call it a "letter of recidivism." It draws on your substance use history, any prior offenses, the treatment and recovery steps you've taken, and your mental health history.
The RMV does not waive this requirement. Until it is completed, your suspension can remain in place indefinitely, so it's worth getting it done early.

How the Process Works
Complete the quick form. Include your hearing date and your attorney's name if you have one.
Meet with your clinician. A one-hour interview by secure video, or in person at our Medway office.
We complete your report. Your substance abuse evaluation and risk of recidivism assessment.
Results go directly to the RMV, so you can move forward with your hearing.
Turnaround: in as little as 48 hours when your hearing is close. Tell us your date and we'll prioritize accordingly.
What the Evaluation Assesses
Alcohol and drug use history, including current use or abstinence
Prior offenses and driving history
Past and current treatment, support groups and recovery steps
Mental health and medical history relevant to driving safety
Standardized screening tools
A risk of recidivism opinion, with recommendations when clinically appropriate
How to Prepare
Have your RMV letter or hearing notice on hand.
Gather any court paperwork, treatment records or program completion letters.
For a video appointment, find a private space with a stable internet connection and a device with a camera.
Answer openly. The evaluation looks at the whole picture, and it is only as useful as the information it is based on.
Because our clinical team includes multiple board-certified psychiatric nurse practitioners and clinical nurse specialists, we can often offer earlier appointments than a practice that depends on a single provider's schedule.
Other RMV Requirements You May Have
Immediate Threat cases often also require a medical evaluation or a loss of consciousness form completed by your primary care provider. RMV suspension hearings are currently held by phone and booked through the RMV Online Service Center. Chemical test refusal hearings are the exception: they are held in person at the Boston (Haymarket) RMV Service Center. Official forms are available on the Mass.gov RMV forms page.
Pricing
Substance Abuse Evaluation with Risk of Recidivism: $450

Frequently Asked Questions
Is a video evaluation accepted by the RMV?
Yes. Our evaluations are approved by the Massachusetts RMV, and the clinical content and reporting are the same whether you attend by video or in person.
How fast can I get my evaluation?
In as little as 48 hours when your hearing is close. Expedited scheduling is available for an additional $225. Let us know your hearing date when you contact us.
Which court handles OUI cases from Warren?
OUI cases arising in Warren are heard at East Brookfield District Court. The RMV evaluation is a separate requirement from anything the court orders.
Do I have to travel from Warren to be evaluated?
No. Warren residents can complete the entire evaluation by secure video. In-person appointments are available at our Medway office if you prefer.
Do I need both a substance abuse evaluation and a psychiatric evaluation?
Some RMV cases require only one; others require both. Check your RMV notice, or send it to us and we'll confirm before you schedule. A combined rate applies if you need both.
Can you evaluate ignition interlock violations?
Yes. We complete RMV evaluations for interlock violations as well as OUI, DUI and Immediate Threat cases.
Will you work with my attorney?
Yes. List your attorney on the intake form and we'll coordinate dates and documents.
What happens to the results?
Findings are reported directly to the RMV following your evaluation, so you won't need to forward anything yourself.
Does completing this evaluation guarantee my license will be reinstated?
No. The RMV hearing officer makes that decision, and the RMV or a court may have additional requirements. This evaluation satisfies one specific requirement in that process.
What if I've moved out of Massachusetts since my case began?
As long as you're physically located in Massachusetts at the time of your appointment, we can complete the evaluation. If you've relocated permanently, contact us to confirm eligibility.
Is what I share confidential?
Yes. Your evaluation is handled under the same clinical confidentiality standards as any other appointment, with the exception of the findings we're required to report to the RMV.
